The following is a registration for the Zoetic Summer Camp, for children between the ages of 6 and 12, which will take place at The Zoetic Theatre (526 Concession Street, Hamilton, Ontario, L8V 1A6) from Monday, July 6 to Friday, July 10 between the hours of 8:00 AM to 5:00 PM. *This camp is not affiliated with or licensed by the Government of Ontario. However, we do meet all requirements for unlicensed camps as specified by the Government of Ontario. Please answer each of the following questions below. If you have any questions, please contact info@thezoetic.ca prior to the completion of this form.
